Providence Medical Center (PMC) has announced the launch of a Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) program that extends the hospital's care beyond its walls and into patients' homes.
The new service is supported through Nebraska's Rural Health Care Transformation Program (RHTP) and establishes Providence as a regional hub for remote healthcare serving Wayne and the surrounding communities.
Remote Patient Monitoring uses simple, easy-to-use home devices — such as blood pressure cuffs, glucose meters, pulse oximeters, and scales — to capture a patient's health readings and send them securely to a Providence care team.
The program is designed for patients living with ongoing conditions that benefit from steady, day-to-day attention — including high blood pressure, diabetes, heart conditions, COPD, and chronic kidney disease.
Providence has been awarded $311,701.87 to support the rollout of remote care, technology, and staff training.
